Indicate which of the following applies:

• Member of PCA and the Lone Star Region

• Member of PCA and of the ___________________________________ Region

• Not a member of PCA

Members of PCA who are not members of the Lone Star Region will be granted a temporary membership in the Lone Star Region for the duration of the event.
