Loan Star Region and PCA Forms
The forms below deal with Lone Star Region administrative matters and event-specific forms, e.g., for club racing, autocross, driver's education, and touring. Links may also be found on the respective activity web pages.
PCA event-related forms are also available on the PCA website and in the Region Procedures Manual.

PCA Event Insurance and Waiver of Liability
Any LSR event conducted under the auspices of PCA that involves moving vehicles requires event insurance and waivers of liability. The waivers must be signed by each person (including minors and observers) participating in the event. This document explains this policy and has copies of the forms.

PCA Guidelines for Observer and Incident Reports
Any PCA-related event conducted by LSR that requires PCA insurance coverage must be evaluated by an observer and reported to PCA. If an incident occurs during the event, a special report is required. This document contains Post Event and Observer Report forms and Incident Report forms and guidelines.

Loan Star Region Expense Report
Expenses incurred on behalf of LSR that are approved for reimbursement are reported on this form. If you have Adobe Acrobat (not Adobe Reader), It can be completed electronically and submitted by e-mail as an attachment with supporting documentation, or it can be printed out, completed by hand, and mailed with the supporting documentation. |

Texas Sales Tax Exemption Form
As a non-profit organization, LSR may be exempt from sales tax on approved club purchases. This form is filed with the vendor to obtain exemption from sales tax, if applicable.

PCA Records Retention Guidelines
PCA Regions are required to retain records for tax purposes and because they relate to the organization or operation of the Region. This latter group includes items such as the letter of incorporation, bylaws, financial statements, and insurance and liability forms, etc.
